The PCB had initially refused Akmal, all-rounder Shahid Afridi and spinner Saeed Ajmal permission to play in the competition at all.
They then reversed the decision on Monday, allowing them short stints in the Australian T20 competition.
Afridi decided to terminate his own deal with Sydney Thunder, saying he wanted to prove his form in domestic cricket instead.
The PCB gave Akmal permission to play in only the opening game of the tournament, and the Sydney Sixers have decided that was not enough.
Genral manager Stuart Clark said: "While it would have been great to have Umar here for the start of the BBL season, the short time frame offered is not in the club's best interest and we have decided to see if a suitable replacement is available rather than continue down this path."
