'The Hammer' joined fellow Stoke thrower Phil Taylor in next month's Winners Group after edging past the Dutchman in Wednesday's final.
He won five of his seven round-robin games to finish second behind Dave Chisnall, then defeated Adrian Lewis 6-2 in an all-Stoke semi-final.
Van Gerwen awaited in the decider and Hamilton quickly raced into a 3-0 lead with finishes of 121, 98 and double six, before missing two darts for the fourth leg.
That allowed the Premier League champion to finally get on the board, and he was soon level after nailing a 121 finish on double 14.
A fourth successive leg followed before Hamilton levelled, but van Gerwen won the next to move to the brink of victory. However, there was still time for Hamilton to turn the tables one last time with two straight legs of his own, taking out 90 on the bull to complete the victory.
"It's great to be through to the Winners Group. I probably learned a lesson from Group One," Hamilton told the PDC's official website.
"Yesterday I was happy with getting to six points because I've struggled here in years gone by, and today I was more focused."
He added: "The last leg showed what the game's all about; big finishes against big players. There have been some great players here for the past two days but two are through and I'm one of them."
