Caldwell is set to go under the knife next week after playing through the pain barrier this season with the problem.
It means Caldwell will be forced out of the Scotland squad for next month's World Cup qualifier against Croatia and he could face a battle to be fit for the start of next season.
"I have been struggling for the past six months with a hip problem and I have seen a specialist, and I am going to need another clean-up operation that I will get next week," Caldwell told Sky Sports News.
"Hopefully that can get me back to full fitness and get me back playing early next season, and I am just looking forward to getting pain free again and doing as well as I can."
