The in-form 25-year-old will miss the key Group A game at the BayArena after sustaining a thigh muscle injury in the 1-0 win at Hertha Berlin on Saturday.
Sam, who has scored seven league goals this season and was a second-half substitute for Germany against England at Wembley in midweek, underwent tests on Sunday morning after picking up the injury early in the game.
"His absence is very bad news for us," said Leverkusen sports director Rudi Voller.
"This is the game of games for us in the Champions League so we have to put everything we have in it even without Sam."
Leverkusen are currently second in the group, a point behind leaders United, who have eight points.
Ukraine's Shakhtar Donetsk are third with five points and Real Sociedad are bottom with just one point.
