Terry has been struggling with a toe problem and he was expected to undergo an operation at the end of the season.
The England international has been playing through the pain barrier, but the club have now decided to bring forward his time of surgery.
It means that Terry will miss Tuesday's clash with Manchester United, live on Sky Sports 1, and the last game of the campaign at Newcastle.
Terry is also likely to be miss England's tour of the United States.
Blues boss Jose Mourinho revealed Terry's team-mates Frank Lampard and Ricardo Carvalho also need minor operations in the summer to clear up niggles.
"We will have in the next week three operations, not big things but small problems to solve," said Mourinho.
"Problems to players that have given everything for us.
"Lampard, Terry, and Carvalho will have small surgeries."
