The central defender was bought in a joint-agreement between PSV and Chelsea in 2004, with the deal being that after two years in Holland he would then move to England.
However, Alex does not meet the criteria to obtain a work-permit and an appeal looks doubtful, on the grounds that he is yet to win a cap for Brazil - despite being named in a number of squads.
PSV now want Chelsea to agree to him extending his stay at The Philips Stadion.
The Blues have to decide whether to keep Alex as their own asset or whether to sell him on, with Italian giants Milan known to be very keen.
Reports in Holland suggest that the most likely outcome is another two-year deal between Chelsea and PSV - after which point Alex may well be primed for a move to Stamford Bridge.
