The pair suffered injuries while on duty for their respective clubs, Everton and Southampton, at the weekend.
Kilbane strained his hamstring in Everton's defeat to Blackburn Rovers and will miss the trip to Poland.
Toffees physio Mick Rathbone also revealed the midfielder could be absent for the club's next Premiership fixture against Wolves.
"Although Kevin was forced to play for 90 minutes the injury is significant enough to keep him out of the Republic of Ireland's game in midweek and probably next week's game with Wolves," Rathbone told the club's official website.
Delap has withdrawn after tweaking knee ligaments in Southampton's 2-1 defeat at home to Bolton Wanderers.
The midfielder initially picked up the problem in the first half and was substituted early into the second period after failing to shake off the complaint.
Ireland coach Brian Kerr had already lost Stephen Carr and Paddy Kenny for the match which will mark Roy Keane's return to the international arena.
