The highly influential midfielder has damaged knee ligaments during a training match with Holland.
Boateng, who has recently earned a recall to the international scene, has suffered what has been described as a 'serious' looking injury.
The Boro star collided with Barcelona's Mark van Bommel and both have suffered.
Van Bommel injured his left ankle and will have scans on Wednesday although early reports are suggesting an eight-week lay-off.
Boateng is definitely out of the friendly with Italy on Saturday, although he will he will have scans before anything can be confirmed.
Boro boss Steve McClaren will now be left to sweat over whether his midfield lynchpin will be sidelined.
Last season Boro's season nearly capitulated entirely when Boateng was injured and McClaren will be keeping his fingers crossed that the news is positive.
Meanwhile, Dutch coach Marco van Basten has called up Kew Jaliens and Ellery Cairo as cover, after both Nigel de Jong and Ruud van Nistelrooy picked up minor knocks.
