Canadian striker Paul Peschisolido capped a memorable comeback from United, who came from two goals down to win the tie, when he scored the hosts' third with just eight minutes remaining in the last period of extra-time for United's third, before own goals from Des Walker and then Robert Page set up a nail biting finish.
It was the hosts who dominated early proceedings and were unlucky not to have gained an early lead through Dean Windass on nine minutes.
Michael Tonge created space for himself on the left flank and whipped in a dangerous right foot cross and Forest keeper Darren Ward's slightest of touches was enough to put Windass off as he slid in at the back post as his effort went wide before having a header saved ten minutes later.
There was a touch of fortune about Forest's first goal as Blades defender John Curtis, running back towards his own goal, inexplicably passed it into David Johnson's path who took a couple of touches before firing past Paddy Kenny, who possibly could have done better with his efforts, for his 29th of the season.
Paul Hart's men doubled their lead 12 minutes after the interval through winger Andy Reid after a quick free kick resulted in right back Mathieu Louis Jean being released down the right hand side to deliver in a cross that was inch perfect for the on-coming Reid who smashed a superb left foot volley in from eight yards out with the aid of a slight deflection.
Warnock's side were thrown a lifeline just one minute later though when Michael Brown's free kick from the edge of the box was deflected in off Forest captain Walker, who had been booked moments earlier for fouling Carl Asaba.
Substitute Steve Kabba then levelled matters with a breathtaking goal all of his own doing after he teed himself up before rifling home a tremendous volley inside Ward's near post on 68 minutes.
The game then became the first play-off game this year to go to extra-time after no winner was found in 90 minutes and United, in their 60th game of the season, found a third when Peschisolido weaved into the box on 22 minutes of extra-time and hit a low shot past the stranded Ward before both captains Walker and Page put through their own nets to set up a grandstand finish but The Blades just managed to hang on to advance through to what promises to be a pulsating final.
