The deal keeps the 22-year-old at the John Smith's Stadium until 2015.
Smithies has been an ever-present this season, helping surprise package Huddersfield climb to second place in their first season back in the Championship, after scoring the play-off final penalty that got them promoted.
"We want to keep our quality players and we're delighted that Alex has agreed to stay for the long term," manager Simon Grayson told the club's official website.
Goalkeeping coach Paul Crichton added: "The kid has got a lot of talent and he's still young.
"He's working very hard at the moment and he's playing well so he's earning his place in the team.
"Alex has coped very well with the step up to the Championship and he'll be working to make sure that his standards don't drop."
