The deal - subject to a medical - comes after the 25-year-old had his contract with The Saints cancelled by mutual consent back in February.
Monk finished the remainder of this past season with Second Division side Barnsley, having initially joined The Tykes on loan.
The versatile Monk can play as a full back, central defender or central midfielder and has signed a three-year deal with the South Wales club.
