Jimmy Juan's late free kick looked to have earned Ipswich a point against the odds after cancelling out Michael Ricketts's first half opener, but Koumas struck in the final minute to earn Cardiff all three points.
Ipswich were reduced to ten men on 13 minutes when Fabian Wilnis was shown a straight red card for tugging back Cameron Jerome in the box and Cardiff were awarded a penalty.
Ricketts took responsibility from the spot but his weak shot was well saved by Lewis Price to keep Ipswich level.
Ipswich came close to breaking the deadlock on 20 minutes when Nicky Forster saw his volley from an acute angle pushed away by Neil Alexander.
Cardiff broke the deadlock on the half hour mark when Joe Ledley picked up a poor pass from Richard Naylor and fed Ricketts who superbly drilled his shot past Price.
The home side almost doubled their lead on 41 minutes when Ledley saw his curling shot kept out by Price.
Ipswich tried manfully to get back into the contest and Luis Castro Sito forced Alexander into a fine save on 44 minutes before Forster volleyed straight at Alexander right on the stroke of half time.
Cardiff dominated the second half and both Paul Parry and Jerome had shots straight at Price as the home side looked for the killer second goal.
Price denied Cardiff a second goal on 65 minutes with a superb save from Ricketts after Jerome had set-up his strike partner right in front of goal.
The Ipswich goalkeeper was proving to be a one man barrier against Cardiff and he made another reflex save to keep out Parry's close-range header on 73 minutes.
Cardiff were made to pay for their wastefulness in front of goal on 86 minutes when Juan struck home a fine free kick from the edge of the box.
The home side threw everything into attack to restore their lead and Koumas grabbed the crucial goal when collecting the ball on his chest inside the box and keeping his composure before drilling the ball past Price.
