Brighton were stunned by Football League new boys Newport County at the Amex Stadium, while Accrington - thumped 4-1 by Newport in League Two on Saturday - upset Tony Mowbray's Middlesbrough.
Two other Championship outfits also fell to lower-league opposition on Tuesday, with Sheffield Wednesday downed by Yorkshire rivals Rotherham and Ipswich knocked out by Stevenage, while League One heavyweights Wolves and Sheffield United were sent packing by Morecambe and Burton respectively.
Charlton, Burnley and Peterborough were in the goals - the Addicks mauling Oxford 4-0, the Clarets defeating York by the same scoreline and Posh overcoming Colchester 5-1 thanks to a Lee Tomlin brace.
In some other standout ties, Bournemouth edged South Coast neighbours Portsmouth, Cheltenham bested Crawley by the odd goal in seven, and 2011 League Cup winners Birmingham snuck past Plymouth courtesy of Kyle Bartley's extra-time winner.
