The two sides are battling it out for a spot in Germany next summer and they could not be separated after 90 minutes in Trinidad.
T&T took the game to their opponents from the first whistle, but Stern John and Dwight Yorke were not at their clinical best in front of goal.
Bahrain dug in and were rewarded with the opener on 72 minutes, Salman Husein nodding in from close range to stun the T&T faithful.
The home side were staring defeat in the face when Port Vale midfielder Chris Birchall fired in a stunning effort from 25 yards to earn The Socawarriors a draw.
The tie is now evenly balanced ahead of the second leg in Manama on Wednesday.
