The former Liverpool, Leicester City and Plymouth Argyle player is reported to arrive at Edgeley Park on Monday for a medical towards a £50,000 move from the Somerset club.
A spokeswoman at County would not confirm or deny to planetfootball.com if the left-back was set to complete his transfer to manager Carlton Palmer's outfit.
The move for Tonkin comes in the midst of the defender upsetting his current club by taking the advice of his agent and not appearing for Yeovil's Conference match at home to Halifax Town on Saturday.
Huish Park boss Gary Johnson told Yeovil's official website that he is very disappointed in the actions of Tonkin and his agent.
Johnson said: "I'm absolutely devastated that Anthony has acted in this way.
"All the players at the club know I would never stand in their way if an opportunity to better themselves comes along.
"To be told by his agent not to speak to the player and that he didn't want him to play today is a disgrace.
"I've even had Gordon Taylor on the phone from the PFA suggesting I hadn't informed the player of the interest from Stockport, which is totally untrue.
"The matter had to be discussed by my chairman and board and to be told the player has allegedly all ready agreed terms with Stockport has really upset me.
"We obviously will wish Tonks (Tonkin) all the best at his new club, wherever he ends up, and I do hope that he will realise that at the end of the day his manager and the club that have brought him threw to this level perhaps deserved a bit more consideration and trust in a situation such as this."
