In a throbbing tussle in the Spanish capital, there was no shortage of incident in an action-packed contest which contained four goals, three penalties and two red cards.
The visitors grabbed the lead in the eighth minute through Javi Moreno's ferociously struck spot kick after Roberto Carlos was dubiously adjudged to have fouled Jose Mari.
Real's prospects of battling back were immediately dented after an horrendously late tackle by Ivan Helguera brought about his expulsion.
But Real hauled themselves level just past the half-hour mark through Luis Figo's clinical finish off the inside of the post.
The Champions League holders stepped up a gear and nosed in front on 43 minutes when Figo converted a penalty after Roberto Carlos was felled.
The tempo showed no signs of abating but when Garcia Calvo was dismissed for a second bookable offence on 62 minutes, Atletico looked dead and buried.
Portuguese ace Figo should have wrapped up the win on 80 minutes when Real were awarded a second spot kick after Roberto Carlos was fouled again but German Burgos produced a superb save.
With Real hanging on, Atletico delivered a rip-roaring riposte in the sixth minute of injury-time as Demetrio Albertini's majestic free-kick was tipped onto the crossbar by Iker Casillas before rebounding off the keeper's head and into the back of the net.
Spanish superstar Raul will be ruing a hat-trick of missed opportunities which he would normally have tucked away while at the opposite end, Casillas twice thwarted the Atletico attack with fine saves.
