Despite the wealth of talent at the disposal of both sides, the game failed to live up to expectations and it was just moment of class that separated the sides.
Manuel Rui Costa threaded a sublime pass through the heart of Inter's defence for Filippo Inzaghi to slot in first time with a superb finish to claim Milan's fourth derby win in the last five games.
Rui Costa was the main difference between the sides as a lacklustre Inter side failed to produce any real chances, with Ivan Cordoba's 66th minute red card capping a dreadful night for Hector Cuper's men.
Milan move above Inter on goal difference into second place, but the real winners on the night were Juventus, who maintain their five-point lead at the top of the table.
Marcelo Lippi's men now have the chance to increase their lead to eight points on Sunday when they travel to Bologna.
In Saturday's other game, Como were held to a 1-1 draw at home against Perugia in a result that does little to ease their relegation trouble.
Nicola Amoruso gave the home side the lead after 28 minutes only for Fabrizio Miccoli to grab a point for the visitors in the final minute of the match, although it could have been all three had Fabio Grosso not missed his 76th minute penalty.
