Both clubs are assessing the damage of their UEFA Champions League forays in midweek, with Inter's Juan Sebastian Veron and Milan's Jaap Stam struggling with knocks.
The former Manchester United team-mates were hurt in midweek and are facing a battle to oppose each other on Sunday.
Veron picked up a calf strain on Wednesday evening as Inter drew 1-1 at reigning European champions Porto.
Inter coach Roberto Mancini withdrew the midfielder from the game as a precaution and will monitor Veron in training in the build-up to Sunday.
If Veron fails to adequately recover from the knock, his place will be taken by former Milan midfielder Edgar Davids.
Stam is a doubt to line up against Inter due to the hamstring complaint he suffered during the warm-up before Milan's 1-0 victory at Old Trafford.
Milan coach Carlo Ancelotti will give Stam plenty of time to prove his fitness before the clash at Giuseppe Meazza but is unlikely to gamble on the centre half unnecessarily.
Ancelotti will field the same back four which started against The Red Devils if Stam fails to make the game, meaning Cafu will retain his place at right back, with Alessandro Nesta, Paolo Maldini and Kakha Kaladze forming the remainder of the rearguard.
