Juan Sebastian Veron's strike six minutes into the first period of extra time proved enough to give Inter their first Supercup success since 1989.
Patrick Vieira made his debut for Juventus and he came close to scoring when he saw his header fly just over the crossbar.
David Trezeguet had a goal controversially ruled out right on the stroke of half time with the Frenchman harshly adjudged to be offside.
Vieira was inches away from marking his debut with a goal just before the hour mark when he thumped the woodwork with a header.
Inter were then unlucky not to break the deadlock when Gianluca Zambrotta got back to clear Adriano's shot off the line.
Alessandro Del Piero missed a golden opportunity to score when he somehow missed an open goal after a mistake by Walter Samuel.
Neither side could find a goal and the game was forced into extra time.
Inter made the breakthrough somewhat against the run of play on 96 minutes with Veron drilling home from the edge of the box after being teed up by Adriano.
Adriano came to Inter's rescue late on when he cleared a Del Piero free kick off the line as Inter held on for the win.
