It appeared as if normal service would continue when Andrei Shevchenko netted from the spot on 13 minutes, but a blooper from Dida allowed Andrea Gasbarroni to level and it proved a costly mistake as Francesco Antonioli kept Milan at bay for the remainder of the game.
Milan now trail Juventus by nine points and the Old Lady of Italian football have a game in hand.
Sampdoria were not overawed by falling behind to Shevchenko's penalty and they secured an equaliser on 36 minutes through Gasbarroni, although the goal owed much to an error from Milan keeper Dida who completely misjudged his catch and the ball flew in off his shoulder.
The visitors could easily have been ahead at the break, as Sergio Volpi hit the woodwork from a free kick and Marcello Castellini's header hit the base of the post with Dida scrambling.
Milan called the tune after the break, with Shevchenko going close on a couple of occasions and Clarence Seedorf clattering a shot against the woodwork, but they could not find a winner.
Siena climbed further away from trouble with a 3-1 win at Palermo.
The visitors took control early on through Erjan Bogdani who added two goals in the second half to complete his hat-trick as Palermo wilted on home soil.
Denis Godeas handed Palermo a lifeline 20 minutes from time, but it proved nothing more than a consolation as Siena claimed the points.
