Schalke's 1-0 success over Arminia Bielefeld on Saturday had taken the Royal Blues three points clear at the summit.
But Werder were quick to reel in their title rivals and pounced late to overcome Wolfsburg at the Weserstadion.
Daniel Jensen's 17th minute goal put Werder ahead, but Wolfsburg drew level four minutes before half-time through Isaac Boakye.
With the clock ticking, Naldo scored with four minutes left to provide Werder with an important three points.
Bayer Leverkusen moved up to sixth in the table after hanging on for a 2-1 success at Borussia Dortmund.
Andrei Voronin scored on 24 minutes for Bayer and substitute Stefan Kiessling added a second on 74 minutes, just five minutes after being introduced.
Martin Amedick pulled one back on 85 minutes, but Dortmund were unable to conjure up an equaliser.
