The former Germany international star has played a pivotal role for the club since arriving at the BayArena and now looks likely to be rewarded for his loyalty.
"We are toying with the idea (of a long-term contract)," Bayer boss Heinz Holzhauser said.
"He is a real leader on the pitch, alongside Bernd Schneider and Robson Ponte, and we want to keep all three.
"That says everything about how important he is to us."
Ramelow, who retired from international football shortly before Euro 2004, feels he still has plenty to offer and admitted that he could take up a role off the pitch when he finally hangs up his boots.
"I would definitely think about it, but I'm sure that I've still got at least five years' football in me," he said.
