Mathijssen joined AZ two years ago from Willem II, but has been unable to make an impact in Louis van Gaal's plans.
After loan spells at RKC Waalwijk and NAC Breda, Mathijssen has now agreed a deal to spend next season with his first club.
The 24-year-old is Willem II's second signing of the week after a permanent deal was agreed with Feyenoord for Maikel Aerts.
The goalkeeper was on loan at Willem II and has now signed a three-year contract with the club after learning he was no longer part of Feyenoord's plans.
Aerts' arrival, and that of Kenneth Vermeer on loan from Ajax, has prompted Willem II to allow Tristan Peersman to join OFI Crete.
The Greek club have confirmed that the Belgian goalkeeper has accepted a two-year deal to end a frustrating time in Tilburg.
Meanwhile, Willem II are also keen on signing Jason Oost on a free transfer after the striker rejected an extension to his expiring contract at Sparta Rotterdam.
