AZ's meeting with Feyenoord was the tie of the round and it proved to be a fiery affair, as the home side had Barry van Galen and Kenneth Perez sent off inside 30 minutes.
Van Galen was dismissed for violent conduct, while Perez was given his marching orders for handling the ball on the line.
Perez's handball gave Feyenoord the chance to open the scoring from the spot, but Henk Timmer saved Dirk Kuyt's effort.
Kuyt atoned shortly afterwards, however, netting a header which set them up for victory.
Danko Lazovic added a second on 51 minutes, but AZ fought back and Denny Landzaat handed them a lifeline with a goal seven minutes from time.
However, as they pressed for an equaliser, Feyenoord countered and Kuyt netted his second of the game to seal victory.
Utrecht's hopes of a hat-trick of Amstel Cup wins were dashed by NAC Breda.
Foeke Booy's Utrecht took an early lead through Dave van den Bergh, but NAC fought back and Mike Zonneveld's effort forced extra time.
NAC proved the stronger in the extra 30 minutes and Yuri Cornelisse netted the winner after 116 minutes.
ADO Den Haag got the better of Twente in a penalty shoot-out.
A similar comment applies to TOP Oss, with the Gouden Gids Divisie side taking the scalp of RKC Waalwijk.
