Jon Dahl Tomasson had given Feyenoord a fifth minute lead at the Phillips Stadium when he lobbed over PSV keeper Patrick Lodewijks after latching on to Pierre van Hooijdonk's flick on.
The hosts were level on 58 minutes when Mateja Kezman fired home just moments after entering the fray as a substitute.
Extra-time failed to produce a winner and it was left to Dennis Rommedahl to be the PSV hero with the decisive spot-kick in the shoot-out.
Feyenoord's Leonardo handed PSV the initiative by missing his penalty but then Wilfred Bouma saw his effort saved.
Brett Emerton stepped up for the eighth penalty of the shoot-out but Lodewijks was equal to it which presented Rommedahl with the chance to send PSV into the last four.
FC Utrecht also booked a place in the semi-finals as they came from behind to beat Heerenveen 3-1.
The visitor's took a first half lead through Erik Edman's free-kick, but it was a set piece from Pascal Bosschaart that squared the tie before the interval.
After the break Igor Gluscevic put the home side in front and Dick Kuyt wrapped up the victory in injury-time.
