PSV were strongly fancied to book their place in the semi-finals after being handed a home clash with NAC in the last eight.
However, NAC were in no mood to lie down and stunned their more fashionable opponents with a gritty display.
Nourdin Boukhari scored the only goal of the game on 24 minutes as NAC held firm to take the victory.
NAC have fashioned a reputation as giant-killers this term, having sent Ajax packing in the previous round.
Utrecht were forced to come from a goal down to beat Gouden Gids Divisie side Heracles Almelo.
Roel Buikema gave Heracles the lead on 20 minutes, but Dave van den Bergh fashioned a response on the half-hour mark.
Utrecht took the lead through Hans van de Haar on the hour, but plucky Heracles drew level courtesy of a strike from Yuri Rose.
However, Utrecht had the last word and secured victory when Stefaan Tanghe hit the target with ten minutes remaining.
Feyenoord crashed out to Twente in extra time.
The game was evenly fought throughout with Dirk Kuyt cancelling out Blaise N'Kufo's early opener for Twente.
Extra time tested the fitness of the players, with Twente showing the greater resolve and pinching the match with two goals in as many minutes through Bas Sibum and Kim Christensen.
