Polak said last month that he was looking for Twente to prove their commitment to him by offering him a suitable new deal.
With the 28-year-old not convinced by Twente's offer, he opted to assess other options and was approached by RBC Roosendaal and ADO.
The Hague club proved to be the most persuasive, with Polak penning a three-year deal on Friday.
After sealing his deal, Polak said: "Once you are asked to play for ADO Den Haag the choice is not a difficult one.
"I was at the game with Roda JC on Wednesday and the ambience was fantastic.
"Football is a way of life in this city and ADO have the potential to be a big club.
"I want to play a part in turning these plans into reality."
Polak, who can also operate in midfield, will link up with his new team in the summer.
