The 40-year-old will take the reins at the Estadio do Dragao on a two-year deal having guided Pacos de Ferreira into third place in the Primeira Liga.
Pereira, 44, will join Saudi Arabian club Al Ahli at the end of June after winning back-to-back Portuguese league titles in his two years on the west coast. He also served as assistant to current Tottenham boss Andre-Villas Boas in the treble-winning 2010-11 campaign.
A statement published on fcporto.pt read: "Paulo Fonseca will be the new coach of FC Porto and will sign a contract covering the next two seasons.
"The signing will take place on Monday and will be followed by a short interview with the coach and president (Jorge Nuno) Pinto da Costa."
A former centre-back who retired from playing while at Estrela Amadora in 2005, Fonseca arrived in the Pacos hotseat following spells with Odivelas, Pinhalnovense and Aves.
