Carrick comes into the starting line-up at the expense of Frank Lampard, who began alongside Steven Gerrard in midfield for the 4-1 win over Montenegro on Friday.
Smalling replaces Tottenham right-back Kyle Walker, whose yellow card against Montenegro ensured he would play no part against the Poles.
Otherwise, Hodgson has opted to stick with the side that performed so well last week, which means Spurs winger Andros Townsend starts on the right flank, despite picking up a slight knock on Friday.
Leighton Baines continues at left-back in place of the injured Ashley Cole, who is struggling with a rib problem.
England know a win tonight will guarantee them a place at next summer's World Cup in Brazil, although they would also secure their place as an automatic qualifier from Group H if they match Ukraine's result away to San Marino.
England: Hart, Smalling, Cahill, Jagielka, Baines, Carrick, Gerrard, Townsend, Rooney, Welbeck, Sturridge.
