The former Manchester City and Leicester man spent the latter part of his year with the Portuguese club on loan to FC Sion in Switzerland, before moving to the Bundesliga outfit for a fee of £250,000.
Fernandes, 26, was one of a host of players signed by Sven-Goran Eriksson at the start of his City tenure in 2007, but was sold by Mark Hughes to St Etienne after making 50 appearances two years later.
Loan spells with Chievo and Leicester followed before a permanent move to Udinese that ended less than a year later with his transfer to Sporting.
Freiburg finished fifth in last season's Bundesliga, qualifying for the Europa League, and manager Christian Streich is looking to further strengthen his squad.
"Gelson is a vastly experienced player, he works hard for his team. We are happy to have him and are out to sign two or three more players this summer."
The German club's director Klemens Hartenbach added, "Due to several circumstances he did not have the best of times at some of his most recent clubs. We are convinced he will fit right into our setup as a strong team-play character."
