Mustapha Yatabare and Dimitar Berbatov exchanged first-half goals but neither side could find the net again inside 90 minutes and the game went to extra time.
Substitute Fatih Atik then grabbed the vital goal with eight minutes remaining and Yatabare sealed the win five minutes later.
The 2009 winners will now face Rennes in a repeat of the final from five years ago while a trophyless season is sure to mount the pressure on Monaco boss Claudio Ranieri.
Guingamp took a shock lead in just the sixth minute when Yatabare was on hand to stab home Steeven Langil's cross from close range.
But Ranieri's men were level before the interval as Fabinho broke down the right and crossed for an unmarked Berbatov to slot home from around the penalty spot.
The second half was goalless but Guingamp had a golden opportunity to take the lead early in extra time as the ball was flashed across the six-yard box but Ladislas Douniama could not get a touch on it.
Atik had only been on the pitch nine minutes when he scored what proved to be the winner and Yatabare's second crowned a shock result.
