Two behind overnight, a couple of birdies in the first two holes of his final round got the Swede off to a flyer but he followed it with two bogeys.
Another birdie on the sixth hole was followed by eight pars as the 28-year-old settled into a rhythm before a four at the par-five 15th took him to 15 under par.
Blixt's attacking tee shot on 17 was rewarded by a nerve jangling 10 foot putt for birdie after his flighted chip had shaved the hole.
And he held his nerve to two putt the final green, his 243rd straight hole without a three-putt, to scoop the big prize.
Americans Tim Petrovic (64) and Jason Kokrak (68) tied for second place on 15 under.
Overnight leader John Mallinger, who led Blixt by two, could only manage a one-over 72 to finish on 14 under in a tie for fourth, while Scotland's Russell Knox enjoyed another good week in closing with a 69 and a 12 under total in a tie for ninth with Gary Woodland.
Blixt said: "It's unbelievable . It's been the greatest two weeks on tour (after his Shriners third last week).
"I haven't practiced after tournaments this time with my family out here, and I haven't done this these two weeks and I've won. Incredible."
