Shivnarine Chanderpaul finished on 87 not out as Derbyshire chased down a target of 152 with three balls to spare.
The West Indian hit seven fours and two sixes in his 70-ball knock and received good support from Wes Durston (20), Wayne Madsen (23) and Albie Morkel (15no) after Chesney Hughes had fallen to the first ball of the innings.
Lancashire had earlier mustered 151-8 after being invited to bat at the County Ground in Derby.
Seamer Tim Groenewald removed the top three in his first three overs before Karl Brown's 50 Lancashire's recovery from 19-3.
Groenewald returned later to complete Twenty20-best figures of 4-21.
The result moves Derbyshire top of the North Group with three wins out of three and provided welcome relief from their wretched County Championship form, where they sit bottom of Division One without a victory.
