The competition is the first to be staged at the London Aquatics Centre in Stratford since the 2012 Olympics, where Couch and Barrow finished fifth in the same discipline.
The pair led after the opening two rounds of the FINA event before the Chinese pair of Chen Ruolin and Liu Huixia eventually secured gold but the British duo were pleased with their performance.
"We really enjoyed the competition and it's great to be back," said Barrow.
"We knew it would be tough against them. The result means we're the leading pair in the Commonwealths though, so that's a big confidence boost ahead of Glasgow this summer."
Their total score of 321.24 was enough for the 2012 European Aquatics gold medallists to secure second place, with Couch delighted to return to action after suffering a dislocated knee.
"I had a bad injury at the beginning of the year," she said. "But we have come back strong and came here today thinking 'let's do it' and the support from the crowd really helped us."
