The prop, 32, who is out of contract at the end of the season, will miss the remainder of the current Super League campaign.
The Australian has endured an injury-hit three seasons since moving to Saints and was once again in the treatment room after damaging his shoulder against Castleford on Easter Monday.
It was hoped that the injury could be cured without the need for an operation but, following a failed comeback attempt in May, has had decided to go under the surgeon's knife.
Saints coach Nathan Brown told the club's official website: "I suppose if he was a younger player he could have had the op earlier on but, as he's older, he wanted to play.
Career not over
"But the injury isn't right and that means it's likely this will finish his year but not his career."
Brown is not too concerned at the absence of Perry, insisting he has plenty of cover in his squad.
He said: "It hurts us a little but we have Alex Walmsley, Tony Puletua, Louie McCarthy-Scarsbrook, Ant Walker and Anthony Laffranchi will be back this week too.
"Greg Richards from the under 19s has a lot of talent and he will begin to train with us soon once he has finished his A Levels."
