The Leeds skipper was taken off at half-time of the 30-10 World Cup warm-up victory at Warrington's Halliwell Jones Stadium and is doubtful for the Rhinos' Super League clash with Widnes on Monday, despite being included in their squad.
Sinfield, who impressed at both stand-off and hooker during his time on the field, said: "I'm not that sure but I'll see the surgeon over the weekend. We think it's dislocated at the moment.
"At this stage I don't see how I can play (on Monday). I think it will need some sort of operation but I will see what the surgeon says."
Hull forward Gareth Ellis, who had a long spell out at the start of the season with a foot problem, was helped off in the second half but England coach Steve McNamara played down his injury.
McNamara said: "He just had a big back spasm. Hopefully it's not as serious as it looked.
"He was in a lot of pain, as you could tell, but he is up and walking about again now at least.
"It'll be 24-48 hours before we can find out the full extent of it."
