Carr, 27, wrote himself into the Connacht record books during his first spell in Galway, scoring a club record 34 tries.
The Ireland Wolfhounds international made a total of 73 appearances during his three seasons at the club before leaving to join Leinster in 2011.
Carr went on to score seven tries in 33 appearances for the Heineken Cup champions - however he has now decided to return to Connacht on a two-year deal.
"I'm delighted to have re-signed for Connacht," said Carr. "I thoroughly enjoyed my three seasons there and I can't wait to get back to the club and continue to improve my game.
"I can see that there is fantastic work being done at Connacht both on and off the field. I'm really looking forward to being part of that again and contributing to the growing success over the next couple of seasons."
Sligo-born Henry - who came through the youth ranks at Connacht - rejoins on a two-year contract after three years at Munster.
Sears added: "We are delighted to have been able to bring Sean back to his home province and have no doubt he will bring real quality to our squad and increase the competition for a place at hooker.
"He was very highly regarded at Munster and it is great news that he sees his future with Connacht."
