Terry captained Chelsea to his first league title - and the club's first in 50 years - following Mourinho's appointment at the club in the summer of 2004.
The academy graduate formed a brilliant partnership with Ricardo Carvalho at centre-back, appearing in all but two Premier League games as Chelsea secured the title with the best defensive record, the most clean sheets, and the most points in history.
Terry's personal contribution to Chelsea's success was recognised by his fellow professionals when he became only the fifth defender to win the PFA Players' Player of the Year award.
As well as his defensive consistency, Terry had scored eight goals over the course of the campaign, including a vital winner in a memorable Champions League tie against Barcelona. "It has been a real special season with Chelsea and this just adds to it," he said after collecting his award.
Terry won a landmark fifth FA Cup in May 2012, marshalling Chelsea's defence expertly as they beat Liverpool 2-1 at Wembley Stadium. "I think this win can help us win the Champions League with the momentum it gives us," he said.
Terry was forced to sit out Chelsea's Champions League final win over Bayern Munich, but in an interview with Sky Sports pundit Jamie Carragher in December 2015, he still described the game as the most memorable of his career.
The defender had been vital in securing Chelsea's passage to the final, and he joined his team-mates on the pitch to lift the trophy after the game. It exorcised the demons of his costly penalty miss in the 2008 final against Manchester United.
Terry was dropped from Chelsea's starting line-up under Rafael Benitez, who felt he was no longer up to the task of playing twice in a week.
But while some observers had written Terry off, he came back with vengeance after Mourinho's return to the club in 2013. In the 2014/15 season, he played in every single Premier League game as the Blues stormed to the title again. He was named in the PFA Team of the Year for the first time since 2005/06.
