The destination of the title will now be decided in Miami on Thursday after LeBron James and Ray Allen inspired the Heat.
The Spurs had lead by 10 going into the final quarter, however Miami hit back to - with Allen's three-pointer with 5.2 seconds on the clock forcing overtime.
Allen then held his nerve in the extra period, with his free throws moving the Heat clear.
Happy
James, who finished with 32 points, 11 assists and 10 rebounds, said on Sky Sports: "[I wanted to] leave everything on the floor. If we're gonna go down tonight, we're gonna go down with me leaving every bit of energy on the floor.
"I gave it my all tonight and we live to see another day.
"It's the two best teams in the league right now and that's why it's a game seven. I'm just happy to be a part of it."
Tim Duncan landed 30 points and 17 rebounds while Kawhi Leonard had 22 points and 11 boards as the Spurs pushed hard for the title-clinching win.
However it wasn't to be and it will now be a winner-takes-all showdown in Miami.
