The Exiles' European debut last week was a nightmare - they were hammered 68-19 by Stade Francais at the Kassam Stadium.
In the backs, Joe Ajuwa, Seb Jewell, Simon Whatling and Gordon Ross all come in to the starting line-up.
Ryan Davis moves from inside centre to full-back, and Ross replaces Gavin Henson at fly-half.
Up front Martin Purdy, who spent two seasons in Italy with Gran Parma and L'Aquila makes his first start of the season.
There's also an entirely new back row with Ed Williamson, Michael Hills and Alfie To'oala coming into the side.
The Italians also had a terrible start to their Pool 5 campaign, losing 56-3 at Grenoble.
Their squad includes prop Guillermo Roan, who played for Welsh 14 times last season, whilst on loan from Wasps.
