Michael Ennis will start at hooker for New South Wales in Wednesday's State of Origin decider after Robbie Farah was finally ruled out with a broken hand.
Farah suffered the injury playing for Wests Tigers against Penrith a week and a half ago and, after undergoing surgery, was hopeful of making a miraculous recovery in time for the crunch clash in Brisbane.
However, he was formally left out by Blues coach Laurie Daley on the eve of the match.
Ennis, who made the last of his seven Origin appearances in 2011, has been with the Blues squad since it went into camp last week after a loophole allowed him to serve a one-match ban for his club Cronulla.
Queensland recall fit-again scrum-half Cooper Cronk after he missed the defeat in Melbourne which enabled New South Wales to set up a decider at a sold-out Suncorp Stadium.
With Billy Slater out for the season with a shoulder injury, Maroons coach Mal Meninga switches Greg Inglis to full-back, moves Will Chambers to centre and brings in winger Dane Gagai for his Origin debut.
