The Repsol Honda rider had been dominant in the morning session but finished the afternoon down in 14th after a nasty spill.
The Spaniard lost control of his bike at the end of the long Mugello straight and slid down the grass verge.
Marquez was taken to the circuit medical centre for treatment but his team later confirmed that he had not suffered any serious injuries.
Back on the track, world champion Lorenzo topped the timesheets with a fastest lap of one minute, 48.375 seconds.
The Spaniard pushed Yamaha team-mate and multiple Mugello winner Valentino Rossi into second place by just 0.034 seconds.
Britain's Cal Crutchlow finished third on the Tech 3 ahead of Ducati's Nicky Hayden and championship leader Dani Pedrosa on the other Repsol Honda.
Bradley Smith, Crutchlow's team-mate and compatriot, was forced to settle for 13th position after he crashed on turn seven and will have an arm injury assessed ahead of third practice on Saturday.
