Chiefs coach Ian Foster has attempted to address his side's slump in form by dropping All Blacks internationals Brendon Leonard, Richard Kahui and Sione Lauaki.
Colin Bourke replaces Lauaki at No.8, with Junior Poluleuligaga taking over the scrum-half duties.
The axing of Kahui means the back division is rejigged, with Callum Bruce moving to inside centre, Jackson Willison slotting in at outside centre and Sitiveni Sivivatu returning on the wing.
Foster has also handed a start to front-rower Nathan White, while lock Kevin O'Neill and James McGougan join Leonard, Kahui and Lauaki among the replacement.
The struggling Cheetahs will be buoyed by the return of Juan Smith and Meyer Bosman to their line-up to face the Chiefs.
Smith makes his return after a family bereavement, while Bosman was rested for the 45-6 defeat to the Crusaders last weekend.
Fly-half Naas Olivier, lock Barend Pieterse, prop Wian du Preez and hooker Ryno Barnes also come back into the line-up.
