Harlequins welcome back Ugo Monye for Monday's capital clash against London Irish at Twickenham, which is live on Sky Sports.
England winger Monye, who has not played for his club since the second match of the season due to injury, takes the place of Gonzalo Camacho.
Camacho himself is now sidelined after picking up an injury during Quins' Amlin Challenge Cup triumph away at Connacht.
Ceri Jones is also ruled out, so boss Conor O'Shea, who will come up against Irish for the first time since he left the club five years ago, is also forced into changes in the pack.
Joe Marler comes into a front row that also contains Joe Gray and James Johnston, while in the second row George Robson returns to partner Ollie Kohn.
Will Skinner is drafted in alongside captain Chris Robshaw and Nick Easter in the back row in place of Tom Guest, who has been given something of a boost with the news that his ankle problem is not as serious as first thought.
Ojo recalled
Meanwhile, London Irish head coach Toby Booth has recalled Topsy Ojo to his starting line-up in a bid to end his team's seven-match losing run.
The Exiles go into the derby fixture having suffered back-to-back defeats at the hands of Toulon in the Heineken Cup, although they have beaten Quins on two of the clubs' previous three Twickenham meetings.
Ojo starts on the opposite flank to Sailosi Tagicakibau, while Chris Malone retains his place at fly-half. In the pack, George Stowers is preferred to Chris Hala'ufia at number eight.

"As local rivals our games always deliver exciting, competitive performances. It should be a great spectacle, playing at Twickenham is always special for our players and supporters alike."
The Aviva Premiership clash, which is expected to gather a crowd of over 70,000, will see South African prop Faan Rautenbach make his 100th appearance for London Irish.
