Seb Stegmann replaces the injured Ugo Monye, while James Johnston and Maurie Fa'asavalu return from international duty with Samoa to start at tight head prop and blindside flanker respectively.
Number eight Nick Easter continues as captain, while Charlie Matthews and Tom Casson return from injury and are named among the replacements.
Worcester have winger David Lemi back in their starting line-up while Scotland prop Euan Murray will make his first start for the Warriors after his arrival during the week.
Lemi is the clubs' leading try-scorer this year and his return is one of just two changes made by Richard Hill as his side visit the reigning champions.
Alex Grove replaced the injured Joe Carlisle and partners Josh Matavesi in midfield as the only other switch, with lock James Percival again the captain.
