Pablo Canavosio will start at scrum-half for Italy in Saturday's Test against Australia in Padua.
The Viadana player, who has 20 Italy caps mainly on the wing, has earned a starting spot at number nine having been deployed as the spare scrum-half on the bench during June's 26-0 defeat in South Africa and the 13-12 win in Argentina.
Among other position changes, Andrea Masi and Andrea Marcato swap their usual roles. Masi moves to full-back having played at fly-half during the Azzurri's Six Nations triumph over Scotland.
Marcato, full-back in the 23-20 win over Scotland, goes to fly-half while Mirco Bergamasco is due to start on the wing despite regularly playing at centre.
Coach Nick Mallett has also named uncapped scrum-half Giulio Toniolatti as a replacement for the first of three November Tests, with Italy also facing Argentina in Turin on November 15 and the Pacific Islanders in Reggio Emilia on November 22.
