South Africa captain John Smit will return to the Sharks front-row for the visit of the Lions.
Smit and Eugene van Staden come into the side, with Tendai Mtawarira and Jannie du Plessis dropping down to the bench.
Injury to Alistair Hargreaves means Gerhard Mostert is handed a start in the second row, with Ross Skeate named on the bench.
The Lions have also made three changes to their starting XV for their clash in Durban.
James Kamana returns at full-back after serving a three-match ban, while Derick Minnie and George Earle are added to the pack.
