Italy's replacement fly-half Carlo Canna kicked a late penalty to secure a 20-18 comeback win over Canada.
Trailing 18-17, Canna nailed the decisive kick from close range in the 72nd minute after a tight match dominated by penalties.
Canna was yellow-carded in the 76th minute for a high tackle on Dan Moor, reducing the Italians to 14 men. But Italy maintained possession and held on for victory.
Gord McRorie scored all of Canada's points via penalties, three of them in the first half which ended 9-9.
Ornel Gega's try - adding to the brace the hooker scored against the USA last week - and Canna's boot saw Italy home and Conor O'Shea now has a record of played three, won two as the Azzurri's new coach.
After losing 30-24 to Argentina, Italy beat the USA last week 30-24 to give O'Shea a promising start.
