The 18-year-old will sign a four-year contract with the Eredivisie champions subject to a medical after coming through the youth system at De Graafschap.
He signed a contract extension with Pieter Huistra's outfit last year but made just a single appearance and will now begin the new season in the Dutch capital.
"Sam is an extraordinary talent," De Graafschap technical manager Roland Vroomans told www.degraafschap.nl.
"We did not extend his contract for nothing last year, there was an interest.
"So we have made a good choice and are now reaping the rewards."
