Germany survived a scare in an unconvincing 2-1 win over Georgia to secure top spot in Group D and automatic qualification for the European Championships.
A Thomas Muller penalty gave the hosts the lead in Leipzig five minutes after the break.
However, they were pegged back when Jaba Kankava caught Manuel Neuer by surprise with a first time volley from the edge of the box after Germany failed to clear their lines from a corner.
Substitute Max Kruse secured three points and top spot in Group D, however, with 11 minutes remaining after he swept an effort into the bottom corner.
Poland claimed second spot after goals from Grzegorz Krychowiak and Robert Lewandowski secured a 2-1 win over the Republic of Ireland in Warsaw, with Jon Walters scoring from the spot for the visitors.
Scotland had nothing to play for after missing out on qualification last week but beat Gibraltar 6-0 in Faro, with Steven Fletcher grabbing his second hat-trick of the campaign.
In Group F, Romania secured automatic qualification after a 3-0 win over the Faroe Islands thanks to a brace from Constantin Budescu and an Alexandru Maxim strike.
The win was not enough to claim top spot, however, as Northern Ireland got the point they needed away to Finland to head to France next summer as group winners.
Hungary had to settle for a play-off place after a dramatic 4-3 loss to Greece in Athens.
Krisztian Nemeth scored twice along with a goal from Gergo Lovrencsics but the home side secured their first win of the campaign thanks to Kostas Stafylidis, Panagiotis Tachtsidis, Kostas Mitroglou and an 86th minute winner from Panagiotis Kone.
Elsewhere, Albania made sure of their first ever appearance at a major finals as an own goal from Kamo Hovhannisyan and strikes from Berat Xhimshiti and Armando Sadiku secured a 3-0 win over Armenia in Group I.
Portugal had already qualified and won the group but finished their campaign with a 2-1 win away in Serbia, even though they rested Cristiano Ronaldo.
Goals from Nani and Joao Moutinho either side of a Zoran Tosic equaliser were enough to secure victory over the Serbs who had Aleksandar Kolarov and Nemanja Matic sent off, the former from the bench.
Denmark will now go into the play-offs in a bid to qualify for France next summer following Albania's win.
